LETTER—Important information—Health insurance
January 3, 2006
Members of the Omaha Police Union, you should
have received your insurance cards from Coventry. If you have not received
your insurance cards, please contact Coventry Health Care of Nebraska, at
866-629-4135 and let them know. The intern will e-mail you a temporary ID
Card.
Coventry Health Care of Nebraska also sent out a letter along with a
Coordination of Benefits Questionnaire (COB). The COB is used to document
any other medical insurance or Medicare that you or a family member might
have in addition to your insurance through the city of Omaha.
The COB MUST BE filled out and
returned to Coventry Health Care by January
20, 2006. You can either mail it or fax it to Coventry Health Care. The
address to return the COB is listed on the top of the form.
We are in the process of placing the COB form on the Union web site for your
convenience should you need additional copies of the form.
All members are asked to complete the form and return it by January 20,
2006.
We also placed the Prescription Drug Claim form on the web site for those of
you that pay for your prescriptions up front and then file for
reimbursement. (retirees).
Coventry will be sending out informational packets the first week of January
2006. The packets will contain a welcome letter, provider directory, a mail
service prescription order form, as well as other information on Coventry
Health Care System. Please take the time to review the information provided
in the packet.
